SCOPE:
Admission of the patient into the acute or chronic treatment centre, recording and recapping of medical and surgical historyObtain patient information and inform billing department according to unit protocolUpdating this information as the need arises according to unit protocolObtain a Dialysis Prescription from the treating Nephrologist or PhysicianEnsure that the dialysis machine has been disinfected before useLining and priming of machines with correct consumablesWeighing the patient and calculating the dry weight and fluid removalMonitoring and recording of baseline vital signs.Carry out all procedures strictly in accordance with NephroCare Standard Good Dialysis Care (ZA-CI-09-03) and NephroCareStandard Hygiene and Infection Control (ZA-CI-09-04) and NephroCare Standard Water Concentrate and Dialysis Fluid forHaemodialysis and Related Therapies (ZA-CI-10-03)Record and Report all patient data accurately using the Database Euclid and TDMSAdministration of prescribed medication as per doctor's ordersDisinfect the machines as per protocolEducation of patient on renal disease aetiology and treatment modes, blood results, support services and dietary restrictionsUpdate all patient files frequently and document all incidents to ensure medico legal requirements are metStock ordering and control as delegated by the unit managerEnvironmental safety and Infection control must be adheredComply with Immunization policy Comply with all FMC SOP'sAny other duties assigned from time to time by the Clinic Head Nurse within the Scope of Practice of the Registered Nurse Required training and education:
University degree or equivalent diploma Registered General Nurse diploma at SANC Required personal competencies:
Respecting the integrity of the patient-physician relationshipCollaborating with care teamsTreating all patients with dignity and respectActing ethically, fairly, courteously, competently, and proactivelyTreating patient safety as a priorityComplying with the law and our policy regarding adverse event reporting and product complaints Functional knowledge:
Knowledge of renal anatomy and physiologyAetiology and Pathophysiology of the renal disease and treatment modalitiesClinical skills must include: Aseptic techniques, Infection control standards, Venepuncture, Vital sign monitoring, CardioPulmonary Resuscitation.Accurate, precise recording keeping, data capturing and monthly qualitative dialysis assessments , disinfection and maintenance record keeping.Maintain a safe and comfortable environmentKeep the equipment, duty room and trolleys clean and stockedCheck daily the resuscitation trolley to ensure that it is in optimal working condition.Administer blood products as per doctors' orders, following protocol and record vital signs and report any adverse reaction.
