Pharm (Clinical Pharmacy)5 – 7 years' experience in a clinical pharmacy environmentHospital experience is non-negotiableRelevant operational experience and application of clinical pharmacy masters in a hospital setting (non-negotiable)Minimum Job RequirementsLead assigned patient service lines and therapeutic programmesIn collaboration with Group Clinical Pharmacist, ensure the adoption of medication utilisation frameworks, standards and system at hospitalsFacilitates pharmaceutical care services, direct patent care programme, medication utilisation systems and care areas to assure medicine utilisation activities are aligned with patient care needs, best practice and regulatory standardsMonitor and review profiles / chart to identify, prevent or mitigate drug related problems, improper medicine use or dose selection, over-indications, medication without indication and treatment failuresImplement stewardship activities and surveillance programmesTrack and evaluate assigned pharmacy programmes at the facility for quality and routinely benchmark against local and international best practiceEnsure clinical pharmacy is an integral part of health-care delivery system by actively collaborating with healthcare providers within the facilityWork within established parameters, protocols and frameworks for medicine use and controlIdentify high risk patients and implement interventions to improve quality and safetyMake appropriate evidence based, patient-centred medicine recommendationProvide pharmaceutical services throughout the hospital facilityEnsure all transactions are recorded and reported on the approved platforms and systemsProvides accurate, safe and timely medicine therapy based on individual patient needFollow the established escalation processes pertaining to clinical pharmacyEnsure recommendations considers cost beneficial initiatives adopted in the group
